Discover Bellafill: The Filler With Results That Last Five Years

Medically reviewed by Dr. Michael Paltiel, MD, Board-Certified Dermatologist | 20+ Years Experience | Last Updated: April 2026

Bellafill is the only FDA-approved dermal filler clinically proven to last up to 5 years. Most hyaluronic acid fillers like Juvederm and Restylane last 6 to 18 months, which means repeat visits and ongoing cost. Bellafill works differently. It pairs a smooth collagen gel with PMMA (polymethyl methacrylate) microspheres that your body integrates as a permanent scaffold, then builds new collagen around them over the months that follow.

The result: immediate volume correction in nasolabial folds and acne scars, plus a long-term collagen response that holds for years. Dr. Michael Paltiel, MD and the team at Adult and Pediatric Dermatology in Forest Hills, Queens, NY use Bellafill as part of a broader dermal-filler program tailored to each patient.

What is Bellafill, and how is it different from other fillers?

PMMA microspheres + collagen gel

Bellafill is composed of 80% purified bovine collagen gel and 20% PMMA microspheres. The collagen gel provides immediate volume correction at the moment of injection. The PMMA microspheres are biocompatible, non-resorbable beads, each smaller than a grain of sand, that remain in place after the gel is reabsorbed by the body.

What happens next is the key. Your fibroblasts, the cells responsible for making collagen, recognize the microspheres as a scaffold and deposit new collagen around them. Over the following 3 to 6 months, this new collagen becomes the structural support for the correction. The microspheres themselves stay put, while your own tissue gradually replaces the original gel.

How your body integrates Bellafill

  • Day 0: Immediate correction from the collagen gel. You walk out with visible volume restored.
  • Weeks 2 to 6: The bovine collagen gel reabsorbs naturally. The microspheres stay in place.
  • Months 3 to 6: Your own collagen builds around the microsphere scaffold. The correction holds and often improves.
  • Years 1 to 5: Results remain stable. Five-year clinical data shows persistent correction with no significant decline.

Why it lasts when HA fillers do not

Hyaluronic acid is a substance your body produces naturally and breaks down naturally. That biocompatibility is why HA fillers are reversible (they can be dissolved with hyaluronidase) and why they fade. Bellafill bypasses that fade because the load-bearing element is not a substance the body breaks down. It is your own collagen, deposited around a permanent scaffold.

“Bellafill is fundamentally different from a hyaluronic acid filler. With Juvederm or Restylane, you are placing a gel that the body will eventually break down. With Bellafill, the PMMA microspheres act as a permanent scaffold, and the patient’s own collagen builds around them. That is why the results compound over months and last for years instead of months.”

What Bellafill treats

Bellafill is FDA-approved for two specific indications, with broader off-label use in trained dermatology practices.

Nasolabial folds (smile lines)

Nasolabial folds run from the sides of the nose to the corners of the mouth. They deepen with age as cheek volume drops and skin elasticity decreases. Bellafill provides structural support that fills the fold and supports the surrounding tissue, with results that hold for years.

Atrophic acne scars

Bellafill is the only filler FDA-approved for acne scar correction in adults over 21. Atrophic (depressed) acne scars are caused by collagen loss during the healing of inflammatory acne. Bellafill lifts the depressed scar tissue from below and stimulates new collagen formation in the scar bed, producing a smoother surface contour.

Marionette lines and chin shaping

Off-label, experienced dermatologists use Bellafill for marionette lines (the vertical lines that run from the corners of the mouth down to the jawline) and for subtle chin and jawline contouring. Because results last so long, careful candidate selection and conservative dosing are critical.

What Bellafill does NOT treat

Bellafill is not appropriate for the lips, the tear troughs (under-eyes), or any area where reversibility matters. For under-eye correction, an HA filler is the right tool, and our practice covers that approach in detail in our guide on under-eye filler results. Bellafill also is not used in patients with active acne, autoimmune disease, or a history of keloid scarring.

Bellafill vs. Juvederm, Restylane, and Sculptra

Filler Material Duration Reversible Best for
Bellafill PMMA + collagen Up to 5 years No Nasolabial folds, acne scars
Juvederm Hyaluronic acid 6 to 18 months Yes Lips, cheeks, tear troughs
Restylane Hyaluronic acid 6 to 12 months Yes Lips, smile lines, tear troughs
Sculptra Poly-L-lactic acid Up to 2 years No Whole-face volume restoration

Many patients use a combination. Bellafill provides long-term structural correction in fixed areas, while HA fillers fine-tune adjacent zones where reversibility or precision matters more than longevity.

Are you a candidate for Bellafill?

Bellafill is approved for adults age 21 and older with moderate to severe nasolabial folds, atrophic acne scars, or both. Beyond the basic indication, candidacy depends on three things: a successful skin test, the right anatomy, and realistic expectations.

The 4-week skin test (required)

Because Bellafill contains bovine (cow-derived) collagen, every Bellafill candidate must complete a skin test before any aesthetic injection. The skin test is a small injection in the forearm. We assess the area at 24 hours, 72 hours, and 4 weeks for any sign of allergy, including redness, swelling, induration, or tenderness. Approximately 1 in 1,500 candidates tests positive and is not eligible for Bellafill. The skin test is non-negotiable; it is not a formality.

Who should NOT have Bellafill

  • Patients under age 21
  • Patients with a positive skin test or known bovine collagen allergy
  • Patients with active acne or skin infection at the planned injection site
  • Patients with autoimmune disease that affects the skin or connective tissue
  • Patients with a history of keloid scarring or hypertrophic scars
  • Patients seeking lip enhancement or tear-trough correction

“The 4-week skin test is non-negotiable. Bellafill is permanent, so we have to confirm a patient is not allergic to bovine collagen before any aesthetic injection. I tell every consultation: if you are not willing to do the skin test, you are not a Bellafill candidate, and that is fine, we will use a different filler. The point is matching the right product to the right patient.”

What to expect during and after treatment

Numbing and injection

Treatment day starts with a topical numbing cream applied for 20 to 30 minutes. Bellafill itself contains lidocaine, which adds a layer of anesthetic during injection. Most patients describe the procedure as mildly uncomfortable rather than painful. A standard nasolabial fold treatment takes 15 to 30 minutes.

Bruising and downtime

Mild bruising, swelling, and tenderness at the injection site are normal and resolve in 3 to 7 days. Bruising can be covered with makeup. There is no medical downtime: most patients return to work the same day. Avoid strenuous exercise, alcohol, and aspirin or ibuprofen for 24 to 48 hours to minimize bruising.

Building results: weeks 1, 6, 26, and beyond

You see immediate correction at the appointment. Some softening occurs over the first 2 to 6 weeks as the gel reabsorbs and the microspheres settle. From month 3 onward, your own collagen builds around the scaffold and the correction firms up. By month 6, what you see is the long-term result, and that result holds for years.

How long does Bellafill last? Results that hold for 5 years

Bellafill has the longest duration of any FDA-approved dermal filler. The pivotal clinical study followed patients for 5 years post-treatment and demonstrated sustained correction of nasolabial folds throughout that window. This is markedly different from HA fillers, where most patients return within a year for touch-ups.

In real-world practice, some patients return at year 4 or year 5 for a small refresh, especially if they have continued to age in adjacent areas. Touch-ups are typically smaller-volume than the original treatment because the foundation is already in place.

Frequently asked questions about Bellafill

How long does Bellafill last compared to other fillers?

Bellafill is FDA-approved to last up to 5 years for nasolabial folds and acne scars. By comparison, hyaluronic acid fillers like Juvederm and Restylane typically last 6 to 18 months, and Sculptra lasts about 2 years. Bellafill achieves this longer duration because of its PMMA microsphere structure, which stays in place while your body builds new collagen around it.

What is the difference between Bellafill and Juvederm or Restylane?

Bellafill is a permanent collagen-based filler with PMMA microspheres, designed for deeper structural correction in the cheeks, nasolabial folds, and acne scars. Juvederm and Restylane are temporary hyaluronic acid fillers, better suited for lips, tear troughs, and areas where reversibility matters. Many patients use both: Bellafill for long-term structure, HA fillers for fine-tuning.

Am I a candidate for Bellafill?

Most adults seeking long-lasting correction of nasolabial folds or atrophic acne scars are candidates, but every patient must complete a 4-week skin test to rule out allergy to bovine collagen before treatment. Bellafill is not appropriate for tear troughs, lips, or patients with active acne, autoimmune disease, or a history of keloid scarring. A consultation determines fit.

How much does Bellafill cost in NYC?

Bellafill cost in the Queens, NYC area depends on the volume needed and the area treated. Per-syringe pricing is higher than HA fillers, but because results last up to 5 years, total spend over time is typically lower. Dr. Paltiel’s office provides a personalized quote at consultation.

Does Bellafill have side effects?

Common short-term effects include bruising, swelling, and redness at the injection site, usually resolving within a week. Rare risks include lumps, granulomas, or persistent inflammation. The 4-week skin test minimizes allergic reactions. Choosing an experienced dermatologist who injects Bellafill regularly is the best way to minimize complications.

Where can I get Bellafill in Queens or NYC?

Adult and Pediatric Dermatology of Forest Hills, Queens, NY offers Bellafill as part of a full dermal-filler program, including treatment for acne scars. The practice serves patients from Forest Hills, Rego Park, Elmhurst, Kew Gardens, and the broader New York City area.
