Masseter Botox for Jaw Slimming and TMJ Relief

Medically reviewed by Dr. Michael Paltiel, MD, Board-Certified Dermatologist | 20+ Years Experience | Last Updated: May 2026

Masseter Botox is one of the most versatile injectable treatments because it does two things at once. Cosmetically, it slims the jawline by reducing the bulk of the masseter muscle (the chewing muscle on the side of the jaw). Therapeutically, it relieves TMJ tension, jaw clenching, and teeth grinding (bruxism) by relaxing the same muscle so it can no longer clamp down with full force. Most patients see jaw slimming around the 6 to 8 week mark and TMJ symptom relief within 1 to 2 weeks. Treatment takes 15 minutes, results last about 3 months, and downtime is minimal: avoid laying down for 4 hours, and avoid alcohol and strenuous exercise that day. Below: how it works, who is a good candidate, what to expect at your appointment, and the most common patient questions, with input from Dr. Michael Paltiel, MD. To book, see Botox in Forest Hills.

Quick Facts: Masseter Botox

  • What it does: Slims jawline (cosmetic) and relieves TMJ tension, clenching, and bruxism (therapeutic).
  • Brands used at Adult and Pediatric Dermatology: Botox and Dysport (most often), Daxxify (newer option).
  • Charged by: the unit, not the area.
  • Appointment length: 15 minutes.
  • Time to see jaw slimming: 6 to 8 weeks.
  • Time to feel TMJ relief: 1 to 2 weeks.
  • Duration: Roughly 3 months.
  • Aftercare: No laying down for 4 hours; avoid alcohol or strenuous exercise the day of treatment.

What Is Masseter Botox and How Does It Work?

The masseter is one of the strongest muscles in the body relative to its size. It anchors to the cheekbone and the jawline and is what closes your mouth during chewing. In some patients, this muscle is hyperactive (it clenches even when at rest) or hypertrophied (it has grown thick from years of clenching, grinding, or chewing tough foods). The result is a wide, square jaw look at rest, plus a long list of symptoms ranging from jaw soreness to morning headaches and tooth wear.

Masseter Botox uses a neurotoxin (botulinum toxin type A) injected directly into the muscle to temporarily block the nerve signal that tells it to contract. Over the next few weeks, the muscle stops clenching as forcefully. Two changes happen as a consequence:

  • Cosmetic effect: the muscle gradually atrophies (gets smaller) from disuse. This narrows the jawline and creates a softer, more oval lower face. Visible at 6 to 8 weeks.
  • Therapeutic effect: the muscle can no longer clamp down at full strength, which relieves TMJ pain, jaw tension, and the clenching or grinding force that causes bruxism. Felt within 1 to 2 weeks.

Cosmetic vs Therapeutic Reasons for Masseter Botox

Most patients come in for one of these reasons (often both at once):

Reason Symptoms Primary Result
Cosmetic jaw slimming Wide or square jawline at rest, masseter visibly bulges when you bite down Narrower, more oval lower face within 6 to 8 weeks
TMJ pain Jaw aches, clicking, soreness on chewing, headaches at the temples Symptom relief within 1 to 2 weeks
Bruxism (teeth grinding) Worn teeth, sore jaw on waking, partner reports nighttime grinding Reduced grinding force, less morning soreness, less tooth wear over time
Jaw clenching from stress Tight jaw at the end of the workday, tension headaches Tension softening within 1 to 2 weeks

Per Dr. Paltiel, masseter Botox is offered as both a cosmetic option (for jaw slimming) and a therapeutic option (for TMJ, bruxism, and chronic jaw tension). The treatment plan and dosing are tailored to which goal is primary.

The Treatment: 15 Minutes, by the Unit

The visit takes about 15 minutes. The masseter is identified visually and by palpation (you bite down so the provider can locate the thickest part of the muscle). Dosing is decided based on the muscle bulk and the goal: more units for visibly bulky muscles, fewer for slimmer baselines.

Charging is by the unit, not the area. That matters because it means dosing scales to your anatomy: a patient with a thick masseter will pay more than a patient with a moderate one, but the price reflects what is actually used rather than a flat per-area rate. Adult and Pediatric Dermatology most often uses Botox or Dysport, with Daxxify available as a newer, longer-lasting option.

“Strategic injection placement by experienced and qualified board-certified practitioners is what produces a natural look without the ‘frozen’ effect. For masseter Botox specifically, dosing is by the unit and tailored to the bulk of the muscle and the goal, whether the patient wants jaw slimming, TMJ relief, or both.”

Recovery and Aftercare

Aftercare for masseter Botox is the same as any Botox treatment:

  • No laying down for 4 hours after injection. This prevents the product from migrating before it binds to the target muscle.
  • Avoid alcohol the day of treatment.
  • Avoid strenuous exercise the day of treatment.
  • Resume normal eating right away. Some patients notice mild chewing fatigue for the first 1 to 2 weeks as the muscle weakens; sticking to softer foods can help during that window.
  • 2-week follow-up is offered at Adult and Pediatric Dermatology to assess the result and add a small touch-up dose if needed.

Side effects are typically mild: brief redness or pinpoint bleeding at the injection site, very rarely a small bruise. The most common transient sensation is the chewing fatigue mentioned above, which resolves as the surrounding muscles compensate.

Cost and How Many Units You Need

Because Adult and Pediatric Dermatology charges by the unit, your final cost depends on muscle bulk and goal. Typical dosing ranges:

  • Jaw slimming, slim baseline: on the lower end of the dosing range per side.
  • Jaw slimming, thick masseter: on the higher end of the dosing range per side.
  • TMJ and bruxism: dosing is usually similar to slimming, sometimes higher if the muscle is very hypertrophied.

The provider will quote you the exact unit count and total at your consultation, so you know the cost before injection. Adult and Pediatric Dermatology offers payment plans (Cherry, CareCredit) for treatments that combine masseter Botox with other cosmetic work.

Who Is a Good Candidate for Masseter Botox?

  • Patients with a visibly wide, square, or bulky lower jaw line, especially when biting down.
  • Patients with TMJ pain, jaw clicking, or chronic jaw tension.
  • Patients with bruxism (teeth grinding), particularly when the dentist has noted enamel wear or a partner reports nighttime grinding.
  • Patients with stress-related jaw clenching and the headaches that come with it.
  • Patients who want a non-surgical alternative to jaw contouring surgery.

It is not a fit for patients who do not have masseter hypertrophy or active TMJ symptoms, since slimming a normally-sized masseter does not create a meaningful cosmetic change.

“I came in for TMJ pain that I had been managing with night guards and OTC pain medication for years. The masseter Botox started to relieve the soreness within about a week, and within 2 months my jawline had genuinely slimmed. I get it done every 3 months now and it has changed my mornings.”

Frequently Asked Questions

How long does masseter Botox last?

Roughly 3 months, which is the standard duration for Botox in any area. Some patients on Daxxify report longer effects. Most patients schedule maintenance every 3 months to keep the masseter relaxed and the slimming visible.

How long until I see jaw slimming?

Jaw slimming becomes visible at the 6 to 8 week mark, as the muscle gradually atrophies from disuse. TMJ symptom relief is felt much sooner, typically within 1 to 2 weeks.

Will masseter Botox change how I chew?

You may notice mild chewing fatigue for the first 1 to 2 weeks as the muscle weakens. The surrounding muscles compensate quickly and most patients have no chewing limitation by week 3. Sticking to softer foods during that window is a common recommendation.

Is masseter Botox covered by insurance for TMJ?

Coverage for therapeutic Botox (TMJ, bruxism, chronic migraine) varies by insurance plan. Adult and Pediatric Dermatology accepts most major insurance plans for medical dermatology and can help confirm benefits before your visit.

Can I combine masseter Botox with cheek or chin filler?

Yes. Masseter Botox pairs well with cheek filler (which lifts the mid-face) and chin filler (which lengthens the lower face) to create a more oval, balanced lower-face shape. The provider will map out the right combination during your consultation.

Are there payment plans for masseter Botox at your Forest Hills clinic?

Yes. Adult and Pediatric Dermatology offers financing through Cherry and CareCredit, and accepts most major insurance plans for medical dermatology indications.
